The $100 quarterly contribution for the July 1, 2012 Benefit Period under the Equity-League Health Trust Fund has a due date of June 1, 2012. If we receive your payment after June 15, 2012, we cannot assure that your eligibility records will be updated in time for the July 1, 2012 coverage. In the event that payments are received after June 30, 2012, you will incur two penalties - one being a $100 late fee and the other being that your coverage will not begin until your $100 contribution and penalty payments are made. You will also have only up to 31 days after the start of the July 1, 2012 quarter, which is through July 31, 2012.
